A focused medical evaluation is a comprehensive 50-minute visit that gives you an opportunity to discuss your medical issues in depth. We will assess each of your concerns or conditions in accordance with the latest research and will then carefully determine the extent to which a switch to a Whole Food Plant-Based diet and lifestyle can benefit you.

Health coaches build in-depth relationships with patients to inspire them and provide the tools needed to follow doctors' recommendations. Beyond coaching, they can offer assistance with recipes, cooking techniques, and tips for making healthier choices when shopping or dining out.
That's why we've launched the Lifestyle Guidance program for anyone eager to advance their wellness journey with the support of our clinical team, alongside lifestyle and mental wellness coaches.
Lifestyle Guidance is a 12-month program aimed at transforming your lifestyle by equipping you with the skills and support essential for a personal, therapeutic health transformation. This program is tailored for those ready to embrace change and seeking the encouragement to achieve it.
This program is an excellent option for out-of-state or even out-of-the-country! You are welcome to visit our practice anytime for an additional cost, but it is not necessary for this program.
Everything included in this program is virtual, and you can participate from anywhere. If you are local, you are welcome to schedule an in-office appointment
What does Lifestyle Guidance include?

During your first visit, we create a space for you to be heard. This introductory session allows us to get to know you better, address your questions, and discuss your goals for improving your health.
In the follow-up visit, we will conduct a lab review. We'll examine your lab results in detail and explain how these indicators relate to risks for conditions such as heart disease, cognitive decline, diabetes, cancer, and more. We’ll discuss ways you can enhance your health through Lifestyle Medicine.
Coaching is an evidence-based practice that fosters accountability and has been shown to yield positive outcomes when working with a Lifestyle practice.
Your first coaching session, scheduled after the initial visit, will build upon the insights gained. The coach will perform a lifestyle evaluation, looking at your food choices, physical activity, sleep habits, mindfulness practices, substance use, social connections, and relationship with nature, all in a non-judgmental manner. This may lead to immediate action steps to enhance your well-being.
The second coaching session, held after your second visit, will focus on developing a long-term action plan aimed at prevention and/or reversing disease, enabling you to lead a more active, vibrant life with reduced stress and fewer chronic issues.
BONUS: You will have access to The Town Doctor’s Lifestyle coach for any questions or concerns regarding your wellness goals that may arise between your two medical visits.
