• A one-time $150 registration fee is applied to each adult enrollee at the time of registration. (Family registration fee max: $300)
• The Town Doctor® membership fee(s) begins at time of registration.
• Child(ren) can be added at the child rate when a parent is registered.
• Fees are billed at time of registration, and all future membership fees are paid on the 1st of the calendar month. Simple & straightforward - it's the way we do things around here.
• Payment of membership fees must be maintained and required to remain an active member and receive service. 
• The membership fee(s) can be paid monthly, every 3 months, every 6 months, or a year. 
• Credit card required to complete registration. I'm sorry, but we are unable to accept pre-paid cards.
• Our fees are simple & straightforward. Easy to read invoices. If you have questions, simply ask. We're always happy to walk you through.

MEMBERSHIP DETAILS

• Lifestyle Membership includes 4 primary care + 4 health coaching visits annually. Additional visits or services can be purchased as needed. Medical visits can be telemedicine or in-office visits.
• There are never any co-payments.
• Discounted pricing on medication, labs, imaging and additional services are offered as benefits to The Town Doctor® members. These are not included in the membership fee.
• Generic (non-controlled) medications are dispensed by The Town Doctor®
• There are local resources available for low-cost immunizations, and routine -screening tests such as mammogram and colonoscopy. 

Women’s Annual - Includes Pap, Pelvic + Breast Exam - $225

Hormone Visit - $300

Hormone Follow up  - $225 

Referrals - $25 (after initial referral)

Form Fee - $50

Established patient - Add’l medical visit:

CANCELLATION AND RE-ENROLLMENT

If requested, a cancellation will be provided requiring 30 days notice. Upon renewal, a re-enrollment fee as well as the current membership pricing at that time applies. Re-enrollment following a cancellation is not guaranteed.
